  • Card Authorized User
    Order a credit or debit card for an authorized user who is not on your account.
  • Dispute a Credit or Debit Card Transaction
    Contact us to dispute a credit or debit card transaction. 
  • Fraudulent Use of a Credit or Debit Card
    Contact us to dispute a fraudulent credit or debit card transaction. 
  • Visa Balance Transfer Request
    Transfer balances to your PNWFCU credit card and consolidate your card debt.
  • Visa Product Change Request
    Use this form if you have a Visa with PNWFCU and would like to change to a different Visa credit card product. 
  • Due Date Change 
    Change your existing consumer loan (excludes VISA and any real estate loan) due date by up to 15 days at no cost! To change your due date more than 15 days from the current due date, please complete the Loan Payment Relief Application. 
  • Financial Hardship Assistance for Current Members
    If you are experiencing a financial hardship and having difficulty making your PNWFCU loan payments, please submit a Financial Hardship Assistance application. We will review your request and respond within 7 business days.
  • Request to Transport Vehicle
    Request permission to transport your vehicle (securing a PNWFCU loan) across State lines, Out of the Country or between Hawaii and the Mainland.
  • Service Member Relief Request
    If you have entered the military or been called to active duty, and have existing loans with PNWFCU that you want to request interest rate relief on, complete this form.
  • Skip-a-Pay
    Skip a loan payment for one month.  A $30 fee will be assessed per loan. Visa, Mortgage, Business, and Home Equity loans are not eligible for Skip-a-Pay.
  • Term Extension 
    Extend your existing consumer loan (excludes VISA and any real estate loan) by up to 12 months to lower your monthly payment!  This will increase your existing interest rate by .10%.
  • Authorization to Release Account Information
    Authorize PNWFCU to release account information to other individuals. Individuals will not be permitted to conduct transactions on account.  
  • Business Membership Application 
    Apply for a Business Membership.
  • Consumer Credit Report Dispute
    Dispute an entry on your credit report that has been reported by PNWFCU.  
  • Courtesy Pay Opt-In
    Authorize us to pay overdrafts on your everyday debit card and ATM transactions. Click here for more information and tips to avoid overdrafts.
  • Direct Deposit Authorization
    Set up direct deposit of your paycheck. Fill out this form and give to your employer.  
  • Member Relationship Agreement 
    Use to set up a new membership if you do not want to apply online, or if you do not have a US Tax Identification Number, or if you are under 17 years old. 
  • Refer-a-Member
    Share this referral card with your neighbors, friends, coworkers, and family. For every new member who joins, you’ll be entered to win $250 for you and $250 for the new member. 
  • Stop Payment on a Check
    Add a stop payment on a check. 
  • W8 BEN
    Certification of member foreign status. More information can be found here on the IRS website
